Those are the extra rails put in the middle of the track on bridges. They are there so if there is a derailment, it might keep the train on the bridge. This was shot reaching up from under the bridge.

This is fake, he is probably just holding the camera from under the rails using a tripod or something because. One it is too steady, and two, why don't we see his feet?
