Bug - Yes, the sub launched missiles have always been what makes the so called "defense shield" all bullshit. They never mention this threat in any proposal of "The shield".
What makes the"defense shield" even more bullshit is the fact that the interceptor missiles do not even work reliably in test scenarios where they know when and from where a missile is coming. How much chance is there to hit an unannounced missile that also may be so unfair as to spit out decoys and maneuover instead of flying ballistic?
When I was a kid the government said that all that was needed to survive a nuclear attack was to "lay down in a gutter and cover up with wet newspaper". That newspaper would dry out in a fraction of a nanosecond, and I don't know how it was supposed to help divert the immense pressure gradient.
I think, they still think this is true. Only now it is "Plastic sheeting and duct tape".

The effects differ according to where you would be in the blast radius. Newspaper can actually be used (even in bomb shelters) as protection if it's thick enough. Of course, at ground zero, there's nothing that wouldn't be vaporized...
Nope, sorry to disappoint you but the old days fighters had a hard time to get to the altitude of the bombers and still have enough fuel to reach them.
